Exit Information
Date of Exit
Defined as:
The date, determined by the service provider, that a participant is exited from the PHaMS program.
Guide for use:
- This question must be answered.
- Record a day (as 2 digits), month (as 2 digits) and year (as 4 digits) of the date that a person applies for PHaMS services. Thus, if the date is 21 May, 2007, this would be recorded as 21/5/2007.
- The date of exit must refer to the day the service provider has determined the participant has exited the PHaMS program. This could be following a period of suspension or inactivity of up to 6 months.
Why is this information needed?
- Provides information on the date from which a participant exits the PHaMS program.

Defined as:
The reason for which a person is exited from the PHaMS program.
Guide for use
- Select only one box
- Record the reason the participant exited from the PHaMS service.
- Where the reason for exit is not specified, please select 'Other' and specify reason in the space provided.
Why is this information needed?
- Provides information on the reasons why applicants are exiting the Personal Helpers and Mentors program.
